include an examination and consultation. Pertinent data will be collected
which allows us to give you a complete diagnosis and treatment plan. In
periodontics we are concerned not only with the teeth, but with the surrounding
bone and tissue supporting the teeth. After your examination we will be
able to answer any questions you may have.
Patient registration and medical/dental history (Part I ) (Part II) forms are enclosed here, you can directly print these forms by pushing "print this file" button. Please complete these forms and bring them with you to your examination visit. Should any questions arise prior to your appointment, please do not hesitate to contact us. It is our sincere desire to always clarify all aspects of treatment. If you find it necessary to change this appointment, please notify our office at least 48 hours in advance.
